This year we are doing an hybrid enrollment for all students. Normally our enrollment is paper forms only. For this school year and onward we are also implementing Online Enrollment. This will be our first year implementing our Online Enrollment process so issues might occur so please bear with us as we learn this new Online Enrollment system. To keep the enrollment process smooth as possible our *Paper Enrollment Forms will be available still to all and for those that aren't comfortable with online enrollment. Please come into our office and pick up an re-enrollment packet! Enrollment will be available on {DATE} and we will have enrollment forms available onsite. The only document a returning student needs to upload (or bring to the office) will be a current utility bill. If you do not take care of your paperwork PRIOR to the first day of school on {DATE}, you may not be guaranteed your same teacher. If you have an incoming 7th grade student, they must have the T-Dap shot prior to the first day of school or they will be delayed in starting school. Also, new law now requires that all students must have had both Varicella shots (chickenpox). If you have an incoming Kinder, you must have the Oral Health (Dental) completed. If you have an incoming 1st grade student, they must have the Health Physical completed.


Online enrollment is now available on our website here on this webpage. In order to do the online enrollment, the parent must have an active email account. You can upload the additional documents listed below or they can be brought to our office beginning {DATE}. No begin date will be scheduled unless paperwork is all turned in! It is important for your student to begin school on {DATE} so they don’t fall behind!

Online Enrollment is for new students only that have not been previously enrolled in Crossroads Charter Academy (AUESD) before.

Returning students please come in and get an re-enrollment packet at our office.

Again paper enrollment forms are provided to any returning or new student(s) if you are having issues or are not comfortable with online enrollment. Both ways of enrollment will be accepted for this school year only.

Before you start Parent(s) must have a current active email address in order to continue the Online Enrollment process! All school related emails will be sent the parent(s) email. If you already have an email you can continue to the online enrollment process on the next block.

If the parent(s) doesn't have an email, one or multiple parent emails can be created here. (Google Email Account)

Please fill out and bring in or upload at the time of enrollment. We need one document for each new student.

Read each document carefully! If you don't have a printer you may come in and print them out here.

If you plan on emailing the documents, keep them under one email per student. If you're enrolling multiple students keep the documents separate in a different email (example: 2 students, 2 separate emails). Please include the student's First and Last Name in the subject line per student! Don't change the name of the documents when submitting.

It is your responsibility to bring these items!  We do not contact the prior school as it will cause a delay.

  • Birth Certificate
  • Immunization Records (Yellow Cards, or School records)
  • Report Cards for Kindergarten through 8th grade
  • Transcripts for 9th through 12th grade
  • Current utility bill, lease agreement, or mortgage statement.
